The type of personal information we collect from you

We will collect personal data such as your name, date of birth, phone number, address, and email address for the purpose of making and maintaining contact with you.

We also ask for details of your GP practice in case of medical emergency, or threat to your vital interests.

Throughout therapy, sensitive personal information may be disclosed by you, and this may form part of your clinical notes documenting our sessions together.

How we store your personal information

All data will be stored securely during our time together and for seven years following the conclusion of therapy. It will then be destroyed in a secure and appropriate manner.

In the unlikely event of a breach, I will notify all parties and the Information Commissioners’ Office within 72 hours.

Approved third parties and cookies

Sharing your data

Everything you say is kept in strict confidentiality. Your data is NOT shared with anyone, apart from the following in exceptional circumstances:

  • If it is necessary to protect your vital interests (threat of harm to self) or the vital interests of others (threat of harm to others)
  • If it is necessary in relation to legal proceedings
  • If it is necessary for administering justice
  • If it is necessary for medical purposes and is undertaken by a health professional or someone who is subject to an equivalent duty of confidentiality.

As a requirement of my profession, I discuss my client work with my supervisor to ensure that I am working safely and effectively. My supervisor is bound by the same ethical and legal frameworks as me and any discussions remains anonymous.

Your data protection rights

Under the General Data Protection Regulations 2018, you have certain rights:

  • You are allowed to see your notes. This is facilitated by making a formal request.
  • Records are kept for 7 years after termination of therapy and then destroyed.
  • Having your records amended (change of name and address)
  • In the case of clients under 18, records are kept for 7 years after the child turns 18.
  • Under GDPR, you can request your data to be erased. However, there are exceptions to this.
  • In the case of counselling records, insurance companies and ethical bodies ask for records to be available for the period of time as outlined above.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
